Must list every current employee with occupational exposure. Last year's names on this year's form is a citation.
Bloodborne Pathogen Training Records — Within 12 Months
Signed and dated by every exposed employee. Training at hire counts only for the hire year.
Hepatitis B Vaccination Records or Signed Declinations
Every employee with exposure risk — vaccinated or declined in writing. No grey area here.
Hazard Communication Program + SDS Index + All SDS Sheets

PPE Written Program + Training Documentation
